Office Policies

OFFICE VISIT POLICY:

All patients will be required to present a valid Driver’s License or Photo ID, current Medical Insurance Card(s) and Co‐pay at EVERY office visit. Patients without VERIFIABLE health insurance will be required to provide a form of payment BEFORE being seen.

CANCELLATION POLICY:

Office Visits: If you must cancel an Office Visit, we request that you please do so AT LEAST 24 HOURS before your scheduled appointment. This allows us to open up space for other patients as well as potential emergencies. You may be assessed a fee if you cancel your appointment the day of. If you do not call, and fail to show up for your scheduled appointment you will be charged a “No Show Fee” of $50.00.

Procedures: If you must cancel a Procedure, we request that you do so AT LEAST 72 HOURS before your scheduled appointment. As procedures take a certain amount of allotted time, there is often a long waiting list of patients that urgently need to have a procedure but must wait several weeks. Cancelling your procedure within 72 hours allows us enough time to schedule another patient in need. You may be assessed a fee if you cancel your procedure the day of. If you do not call, and fail to show up for your scheduled procedure you will be charged a “No Show Fee” of $100.00.

INSURANCE CLAIMS/BILLING POLICY:

North Shore Gastroenterology Associates, P.C., participates with most major insurance carriers. As a courtesy to our patients, we will file claims for those insurances with which we participate. Please note, that any amount not covered by your insurance company is ultimately your responsibility. While a list of insurance companies we participate with is available on our website, it is strongly recommended that you contact your insurance company prior to your visit to confirm that we are still participating.

PAYMENT POLICY:

Payment will be requested and is required at the time of service for all services that are non‐covered or determined to be the patient’s responsibility, including co‐payments. Payment may be made by check or credit/debit card. If you have any questions regarding insurance, billing or our fees, please call our billing department.
